Tooth Town’s clinicians operate at the hospitals below. Your confirmation email tells you which one. Each hospital has its own admission form, its own fees and its own way of contacting you — this page collects what you need for each.
What every hospital needs from you
- Admission form — complete online at least 48 hours before admission (links below). You will need personal details, next of kin and emergency contact, your email, your Medicare card number, health fund details or work-cover/third-party insurer if any, and any pension or health-care card.
- Hospital fees — if you do not have hospital cover, the hospital invoices its day-surgery fee about two business days before admission. If you do have cover, check your fund’s agreement with that hospital; some hospitals have no full-cover agreement with any fund, so there may be an out-of-pocket amount.
- Anaesthetist fees — the anaesthetist’s rooms email an estimate about five business days before surgery.
- Anaesthetist contacts — which anaesthetic group looks after your child depends on the hospital and the clinician. Their rooms will contact you directly before surgery. If you need to reach them and do not have their details, call us on (07) 3398 5885 and we will point you to the right rooms.
- Estimates — to request a quote, hospitals and anaesthetists ask for: child’s name and date of birth, procedure date and length, the surgeon’s name, the item numbers from the treatment plan, health fund and Medicare details, and the anaesthetist’s name. Call us if you are unsure of any of these.

- Health funds: some funds now have a full-cover agreement with Herston, but this varies by fund and policy. Please check with your insurer before surgery so you know whether an out-of-pocket amount applies.

On the day
- Dress your child in comfortable, warm clothing, and bring a change of clothes if they are not reliably dry at night.
- Bring quiet toys, books or a device for the wait.
- Keep your phone switched on and stay contactable while your child is in theatre — occasionally the clinician needs to reach you during the procedure.
- Once you arrive, your child has nothing further by mouth unless the hospital team says otherwise.
- If your child is unwell, do not present for admission — call Tooth Town and the anaesthetist first (see General anaesthesia at Tooth Town).
